Request your Philadelphia immunization record

Philadelphia keeps shot records in PhilaVax, the city's Immunization Information System run by the Philadelphia Department of Public Health. Your PhilaVax record shows vaccines you received in Philadelphia. You can get a printable record at myvaccines.phila.gov if you can access the phone or email your provider entered, or ask for it by email or mail using the online form or a request form sent by mail, email or fax. You will need to upload a copy of an accepted ID. You can request your own record, or a child's if you are the parent or guardian of a child under 18. Bring every vaccination record you have and ask your chosen office which records it needs.
