Request your Texas immunization record

Texas keeps shot records in ImmTrac2, the state immunization registry. To request your record, the Texas Department of State Health Services says to complete the ImmTrac2 Authorization to Release Official Immunization History form and return it to your health care provider or local health department, or email it to immtrac2@dshs.texas.gov. DSHS also lists a separate ImmTrac2 Adult Consent Form for people 18 and older. Bring every vaccination record you have and ask your chosen office which records it needs.
